Awhile back I deleted all zones except the US. Now I have decided to ship internationally and add all countries back and have totally broken something. Can someone help me with defaults for zones and zone definitions? The changes I have tried to make have disabled the shipping calculator and the shipping estimator does not work. During checkout once you enter United States under the country field an additional box appears under the state asking for the user to enter the state in that field. Customers cannot checkout unless the order falls in the free shipping range.

Under zones I have United States zone code 22
Under zone definitions I have United States 48 states + DC and the rest of the world
Under countries I have United States ISO code (2) 22 ISO code (3) 2

I called my webhost and asked them to restore my site to an earlier time and that did not help. Any ideas?
