Is it possible to require confirmed addresses through the old Paypal IPN?

I don't know. I haven't used it in years. It wouldn't be the solution anyway, since IPN (Standard) can introduce another, more serious problem (no order information from Paypal).

It should be possible to remove thet Express shortcut button from the shopping cart page using something like this in the stylesheet:
