Table Rate doesnt always work

I have had my Table Rate set up for almost two years. In the past month two orders have come through with $0 shipping. I have double checked that the items ordered had weights established, and they do. My Table Rate is based on weight. I'm not sure what to look at next. This is a real challenge with my customers!
