Product Review Reminder

During the process of my store upgrade to 1.5.5e, I downloaded a mod called Product Review Reminder.
I am having a few glitch in testing it but I cannot find a thread for it, and I can't even find the mod in the plugins list so I have no clue where I got it from (or maybe if it has been removed from the list?).

The readme file mentiones this:
"Plugin developed and tested on Zen Cart v.1.5.5e" so it must be quite recent.

When I tried to run the SQL, I got error messages saying something about key already existing. I didn't take note of the exact messages assuming it would be in the logs but it is not. Now, it looks like it is only partially installed and no setting is available.

Can someone point me either to the support thread for this plugin or otherwise help with this installation?
