UPS module shipping not working, URGENT..Please help!

Hi I installed the UPS shipping module to a zencart v1.5 from this link:

this module was created in 2011 so update might be needed
and the shipping estimator show $0 because it dot not picking up the weight of the product eventhough the product weight is set up correctly. Please see the shipping file below and please let me know what I can do to fix this. I need desperately some help. Also I attached the screen shop of the shipping estimator.
