usps shipping module installation problems

I am installing the latest version USPS shipping module into zen cart 151.
After in install the include folder I go to Admin>Modules and I now have
United States Postal Service - Nothing has been selected for Quotes.

Also at the top in yellow is a Shipping Configuration error.
"Warning: USPS shipping module is either missing the username, or it is set to TEST rather than PRODUCTION and will not work."

Clicking the edit button doesn't allow me to do anything.
I have checked with the post office and their server is in production mode.
I have checked with my hosting site and CURL is working.
How do I configure the module.

Admin->Modules->Shipping
click USPS
click Edit

What happens?

The line United States Postal Service has in red - "Nothing has been selected for Quotes" in red.
I click USPS and to the far right a green box with information that has buttons for edit and remove.
I click on edit and the green information box goes away and it looks like the like the first screen only with out the
green information section and the screen goes all the way across.

I have a jpeg attached of what the screen looks like after I click on the edit button.

How do I configure the module.

Is there a reason that you have not entered the USPS UserID (UserName) provided to you by the USPS into the proper field? And then selected the USPS classes that you offer as a selection to your customers?

I am never given a opportunity to enter any information.
I install the usps module and when I click on edit to enter my information it goes to the screen I showed in my
post.
If there is somewhere else I need to input it I don't know where that is.

Go to your Tools ... Install SQL Patches ... and run this ...

delete from configuration where configuration_key like 'MODULE\_SHIPPING\_USPS\_%';

Now see if you can go to the Modules ... Shipping ... and install and configure your USPS shipping module ...

Still the same. I am unable to enter any information when I click on edit.

If you look in the directory /logs ... do you see any myDebug-adm-xxxxxxx.log logs created?
