SQL Error in admin product attributes areas

upgraded from 1.3.9
upgraded by hand using procedures
plugins: bluepay_an, freecharger, ups, usps
Installed via FTP/zc_install
PHP 5.3.29
MYSQL 5.5.39-MariaDB
Started - not sure, only found it today

Store's been live for years. I am looking at raw sql files, there's nothing in products_attributes table so the problem might always have been here.

Customizations:
removed .html from seo urls
added google analytics javascript
changed credit card info so admin can delete info from store database
added "tax exempt customer"
added ultimate seo urls
added facebook ad tracking pixel
added perfectaudience ad tracking pixel
upgrade favicon image link statements
added master admin login (customer's user name, admin's password)
