Could you go to Modules ... Shipping ... and click on United Parcel Service XML upsxml and cut & paste the settings in the right hand panel to your next post?

So far, providing the order is over $100.00 I have Insurance added to the shipping cost ...

I just do a quick test of the shipping with an order over $100.00 with the setting:

Enable Insurance
Do you want to insure packages shipped by UPS?

True
False

Set to false ... then try the same quote with the setting set to true to see the difference ... and it seems to work fine ...

i have both ups and upsxml activated to tell me when insurance kicks in

For testing, can you turn off FedEx and Advanced Shipper?

Also if I add 4 to my cart, I see 4lbs in the shopping_cart but UPSXML appears to be using 11lbs any ideas on why the difference there?
