Login Page Repeating Over and Over

I'm having an issue where the login page is repeating over and over. Below is a screen shot. I assume it's something in the database since I have move a clean version of Zen Cart in with no modifications.

Do you have the same issue with the admin?

No I don't, and it only does it on login page. I've cleared the cache folder and the sessions in the database.

Any modules installed? Any errors in the logs/ folder?

Is there a reason you are not using an email address in the screenprint posted?

No log files at all.

modules appear to be all the basics, order_total, payments, shipping.

That is all that is showing under modules.

Wait, I think I misunderstood what you were saying. I thought you were saying that it's running a continuous loop when trying to login.

When in fact your saying it keeps repeating it self multiple times on the same page.

Check you includes/modules/login/ what files are in there, did someone change a file and not remove it like having 2 header_php.php ? Check the includes/templates/YOUR_TEMPLATE/templates/login_default.php is that code in there twice?
