I cannot seem to locate the fields that are used to register new forum users when the Enable phpBB linkage? = true.

That functionality has been removed from the base Zen Cart handling in v1.5.5. There's an overhaul required for the Bulletin Board Integration plugin (see its support thread for details) to operate in the Zen Cart 1.5.5 environment; I've got no ETA at present.
