You might want to check your files. You may have forgotten/missed changing the YOUR_ADMIN to your current admin folder.

TEXT_NOTE_UNSET_LIST is defined in YOUR_ADMIN/zca_bootstrap_colors.php,

The files are all there and in the correct location.
As a test, I even deleted the file zca_bootstrap_colors.php, and when I go to Tools>ZCA Bootstrap Colors, it just stays on the admin index page. I then add it back, and the page is there but still blank below the Notes.
In a strange twist, the store in question is on a Bluehost server, but I have a test version on Hostinger, and it works fine there.

looks like a typo try changing TEXT_NOTE_UNSET_LIST to TEXT_NOTE_NOTSET_LIST in zca_bootstrap_colors.php
