With Bootstrap 3.7.9 and Zen Cart 2.2.2, it looks like $name is getting set to 'main_page' and is showing up as the pre-filled value on the Contact Us form in the Full Name field. Didn't see any obvious places where this might have been done.

Hmm... happens with template_default as well - it's not a bootstrap template issue.
