Need additional Customer Info

I urgently need to have my client's wedding date, in-hand date, and number of invitations they anticipate order, when they order a sample from the site.

Can I make an attribute to caputre this and put it on my sample page, and then make it mandator

Or

Can it be please on checkout?

What files are in play?

You could look at the way the comments box is made and carries through the checkout- look for files that are prefixed with "checkout_"
