testimonial manager sidebox error

I just installed the testimonial manager from Dehaw to a fresh installation of
zencart 1.3.0.1, but now I am getting this error:

1146 Table 'sonny1_booksjapan.TABLE_CUSTOMER_TESTIMONIALS' doesn't exist
in:
[select * FROM TABLE_CUSTOMER_TESTIMONIALS WHERE status = 1 ORDER BY rand() LIMIT 1]

Sounds like you didn't install the file that defines the TABLE_CUSTOMER_TESTIMONIALS value.

maybe someone can help me.
If i run the sql patch i allways get the return:

1136 Column count doesn't match value count at row 1
in:
[INSERT INTO TSconfiguration_group VALUES (NULL, 'Testimonials Manager', 'Testimonials Manager Display Settings', '1', '1');]
If you were entering information, press the BACK button in your browser and re-check the information you had entered to be sure you left no blank fields.

what does that mean? And what can i do?

Is this a fresh install of Testimonial Manager or an upgrade?

Did you copy and paste the sql statement into admin -> install SQL patches and then run the statement from there?

New install of testimonial manager, clicking on any of the links -- read more, all or add a testimonial returns a page not found error. Zen-Cart version 1.3.8a currently running on machine. Can view the testimonial in admin.
