I installed it, using the same structure in which it came and it installed smoothly, and works great with one small hitch. When someone completes the RMA Form the email that comes thru to me only includes the customer's name, order number, item number and amount. The other fields are blank. It does dispatch the appropriate emails to the customer and to the admin though. I hope this post is of help to you all and maybe getting someone to this point you can get me to the finish line.

The admin e-mail you receive should look similar to this one:

From: Customer Name

Item Name: Test Item
Action Requested: Return

I also noticed that when logged in and you go to the return authorization page the customer information fields are not automatically filled inas they should be.

Are using the latest version 2.2.2?
