I'm having a similar problem - only I'm receiving an email but all it says is:

$EMAIL_MESSAGE_HTML

Nothing else.

The cart I'm installing this on is version 1.2.7
I installed this same mod (or least a version of it) on another 1.2.7 cart months ago and it works perfectly.

I got the mod from a thread here in the forums. (The original one I used that works came from the OLD downloads section)

Any ideas as to what I've done wrong?

Here's where I downloaded the return mod I'm trying to install: Contribution Key

Both of these versions indicate that they are for version 1.3+.

I'm not sure where you can find a version for 1.2.7.

Okay ... I located the original 'working' zip file (version 1.2.*) for the returns mod on my old computer.

I removed the new mod and installed the old one. I'm still getting the same message in the emails. Is there something that could have been altered by the new mod that I've missed and forgotten to remove?

I've checked to make sure my regular 'contact us' emails are working and they are.

This is driving me nuts!

Fixed it!
For future reference to others who may have this problem:
In the Admin, I had the "Use MIME HTML when sending Emails" set to 'true'.
I changed this to 'false' and now the RA is working just perfect.

Just hope changing that setting didn't mess anything else up that I don't know about yet!!

Has anyone found the fix to get this to go to the success page after the customer submits instead of the form just refreshing?

Also, do you notice that now, if you try and reply to the e-mail, it puts the store e-mail addy rather then the customers? It didn't use to do that on 1.2.7. Has anyone found a way to fix that yet???

I have a similar problem in that the customer gets an email after completing the form, but the store owner does not.

Emails appear to be working fine for orders though....
