Login Profile changes

I'd like to add a how you first heard about us to the new customer profile log in.

i swore i found it when i did a search last week but i can't find this at all now

i'm trying to change the following
1. main title "were you referrded to us?" to "How did you find us?"
2. how do you hear about us? to select one
3. make a detailed thing were customers add details to their selection, i have a referred code in that spot and i don't like it
