Customize Invoice Template PDFOC

I hope somebody can help because @zencart.nl PDFOC is not supported by the author (Grayson) anymore :-(
At the moment i tried to change the layout of the default invice template, but it was not really successful. I always get 4 pages instead of 1...
Please see the attached file, thats the layout i want to have.
First the logo, full paper width. Below the logo at one line store store name and so on. Then on the left follows the shipping adress. Some rows later, invoice number and order date. Finally the product table, but i dont really need all columns.
At the moment there is shown:

products Model tax Price(ex) Price (inc) Total(ex) Total (inc)

Is it possible to change it? I need the following layout:

Qty Product Model tax Price (inc) Total(inc)

please see the attached word document, thats the aim :-)
The invoice.php is the default template.
I hope you can help...

Whats wrong? Is it really so difficult to modify this???

I changed ours so that the order was printed on our label invoice paper for the shipping address etc to appear in the right place.

it is a confusing layout - but after a bit of fidling you soon work it out.
