Tax exempt module

I am having a problem with the tax exempt module not removing the tax amount from the purchase.

I did a little playing around with it, now this is what it's doing:

Subtotal: $271.99
Shipping: $6.89
Shipping insurance: $1.40
Tax: $16.32
0: $0.00
Tax Exempt: -$16.40
Total: $280.20

now i have a few problems instead of just 1.

1) The system is now taking the tax exempt out, but it is including the shipping insurance in the tax exempt amount ($0.08).

2) what in the world is up with the 0: $0.00. I looked at the page source code and it is a duplicate of the ottax with the title of 0: and amount of 0.00

any clue as to why this is happening?
