How do I setup this shipping config?

I've looked at several modules and I can't figure out how I can setup shipping in this configuration.

Canada shipping
----------
$7.49 per order* for BC, AB, SK & MB residents.
$9.99 per order* for all remaining provinces and territories.

US Shipping
-------------
$7.99*

International
--------------
$9.99*

* Note: includes shipment of up to two pieces. For each additional piece (over two), please add $2.50 per item. <== Applies to all locations

enable your zone rates. admin-modules-shipping-zone rates

I looked at this before.

However I see multiple problems with it.

1. It appears to support only country level zones. I need province level for Canada.
2. I see no way to add 2.50 for each additional quantity other than listing every possably quantity. Is there any way around this?
3. It only apperars to support 3 zones, I need 4.

It is a starter module. You will need to mod your shipping file to add the 4th country. If you post in the shipping and payment modules forum you may get someone that can explain how to mod your file.

Zones can be configured for as many as you need ...

Save your settings then remove it ...

Now edit the file:
/includes/modules/shipping/zones.php

Read the instructions and modify it for how many zones you need ...
