If you have a contributed order total or payment module and you upgrade from scratch, you may see this problem. All it means is that you haven't yet copied the files ot or payment module files over to your new directory structure. You can either do this, or with your old software, de-install the contributions, and your page 2 will appear. You can then re-install the contribs in your new software at your convenience.

Saw this when I was putzing around with some update stuff, and I wanted to provide this clue in case anyone else got stuck here.
