Nothing changed and I checked my zip code, it's set. I've been on the prod server at USPS for months.

So - figured out my issue. Maybe this will help others.
It was only international addresses. The reason was the order was for more than 4lbs of goods and this is what the USPS limits air letter rates and global priority packages at. You can't send 4lbs+ those ways. I had to open up Air Parcel, but that was $25 for the shipment.
Anyway, if you have an issue with some customers, check the order weight and shipping methods you have active. Try activating all the shipping methods in the USPS module and see if the problem goes away.
Thanks for posting the update that you found USPS can only do what USPS allows a package to weight and the type of shipping limits based on their rates and weights charts ...
Zen Cart cannot control that, only allow the rates that USPS will return to us ...
Another one of those silly things where the code will only do what it is allowed to do and not follow what you thought it would do ...![]()

One of the workarounds I utilized was to re-adjust the weight ratio in
Admin-->Configuration -->Shipping/Packaging-->Package Tare Small to Medium - added percentage:weight. Requires some forethought in your product weight, actual weight, add the tare weight of the package, and do some basic calculation, it should arrive within the correct pricing.
Alright, I installed the USPS module a couple months ago, I could never get it to work, so I went to table rate. But I thought to try it again.
I started fresh. Turned on the USPS module. Registered with USPS, got on the production server, made sure the module was set correctly, I plugged the Id in I was sent (the password is the same as ID I was told). Checked shipping/pkg zip.. is set to 48655 same as I registered with.
Still doesn't work, the table rate takes over. Went to store Admin, address listed as PO box with different zip, figured that shouldn't matter. Went and turned off Table Rate, ran a test through the cart. No shipping came up. Reinstalled Table Rate for time being. Now what?

I appears that it's working, but only for other users. When I got up this morning, I had an order. But when I looked at shipping, it was USPS charged at book rate! This is a pkg. of Dog treats, over 10 lbs and they were charged $5. OHHHH NO. I called her and explained, I need to do some work on this. I went in and made the same order, but of course Table rate came up for me.

I am having the same issue too. The shipping is not working on both of my sites. They were previously working for over a year now so they are in production. I have resinstalled the module and set the debugging. The email just talks about what you can ship. Here's an example.

I installed the Jan 11 patch and the rates are working but now I get this error in the shopping cart.
-2147218052 - The ounces field must be less than 5 digits

I was having the same problems, but I called USPS and they just had to change over to Production from test as mentioned above.
You may want to call USPS and make sure you haven't been switched back to test for some reason or another.
Since they switched me over to production it works great on 1.3.9
