    Could be. I picked up a little HTML over 10 years ago, and that's about it. I don't even know what a div tag is!

    But I had problems getting it to work, so I downloaded some PHP editors, and both reported an unclosed div tag. So I closed it and now it works.

    I have not tested the referenced mod in 139h, however, I have used it as a basis to creating several diverse forms, some of which include the upload facility discussed in this thread - all of which work on 139 without problem. I recommend you read this entire thread.

    Another form worth comparing is the Return Authorisation mod

    I am having trouble getting the form that I made show up on a new page. I even tried to create a new page and make a simple form using HTML, how can I get it to load onto a page that was not created by default by zen cart?

    To be specific, I am getting a 404 error - when I thought I created all of the right files. I even edited the HTML using the define pages editor (so I got my new page to appear in that drop down box, but the page wont load).

    Edit #2, just found the Wiki.. Looking at it..

    thanks for this great addon! I am using the "become_dist_v1.03.zip" version.

    I was wondering if there would be a way to add a "upload picture" feature and have the photo emailed as an attachment with the rest of the email.

    I'm trying to hack my way through this but haven't been succesful. (not a php expert)

    I need some help with fixing the formatting of this form:

    I added the phone number contrib, and it seems to have worked fine, but it's formatting is messed up. Can someone tell me why it doesnt line up with the other entrys? I'd also like for it to be the same length. Once I get this resolved, I feel pretty confident I can add other fields, by duplicating the new phone entry lines in the files and changing the names.
