help debug really weird problem

I'm upgrading a previous shop v1.3.02 > v1.3.6
I'm starting from scratch and have re-built the sites design and imported all of the categories and products.

I've installed allot of add-ons and everything is working great apart from when I click to enter some top categories where instead of seeing the category name at the top of the page I see the default welcome and I don't know what could be causing this.

Only some categories are affected and if defined it also shows the main page from the define pages editor.

Any ideas as to what might be happening ?

Do you have both Products and Categories in your Categories such as categories_id 128 Audio / Optical
