Club dues/T-shirt order form

I'm wondering if anyone has setup something like this for Zencart. Have a club, we have a 3 tier membership, say basic, advanced,expert... basic is just a years membership for $1.00, advanced is a years membership but you also get a hat for $2.00 expert get membership, hat and t-shirt for $3.00... (price examples :) )

Does anyone have a setup like this? So if you choose the expert level you would be able to choose the shirt size.. Also shirts, hats, etc would be sold outside of this as well...

Any info or pointers to info on this would be greatly appreciated...

I assume that your memberships have been created as products that can be purchased...

If so, add the appropriate additions as attributes that require size selection.

Well.. no not sure how that is done? If there a How-to or a FAQ that covers that? Thanks for the help...
