Was this an upgrade or a clean install?

It's a clean install.

... also which message appears appears twice? Are the two instances the same

I have set the firstname, lastname, email, tel., and a number of other fields to be required, the messages repeated are exactly the same. E.g. "Please choose a salutation." appears twice.
