Create Account - Same Alert Message Appearing twice

When required field in the Account Creation page is left empty and the "Submit" button is clicked, the same alert/warning message is displayed twice. Any idea where the problem may be? TIA.

... also which message appears appears twice? Are the two instances the same or similar (the two name messages are almost identical)? Is this in English or another language pack?

It's a clean install.

I have set the firstname, lastname, email, tel., and a number of other fields to be required, the messages repeated are exactly the same. E.g. "Please choose a salutation." appears twice.

Although there are admin switches for including (or not) some of these fields on the form, there are none for making them obligatory. That's determined in the code. What did you do to make them "required"? For example, what changes did you make to "firstname"? Also is there a URL where this can be seen?
