I am having this issue too. How can I fix this? I have tried the other header_php.php mentioned above but it did nothing....

Out of curiosity did you try to install this using a fresh install? Without seeing a particular error there is no way for me or anyone else try to resolve your problem! Do you have an error log with details that can show the possible reason it is failing?

I have other plugins installed but everything was fine till I added the hide categories module.
