Contact Us page is doing something funny. Help!!

Hello everyone,

This is my first time posting in this forum, so please excuse me for my errors.

I am at my end's wit here with my Contact Us page. I did manage to change the text to what I wanted but there seem to be some "extra" text being inserted. I want to get rid of these text, but I just don't know where to look. I've looked everywhere, try change bits here and there as suggested, but to no avail.

The bits that I dont want is Store Name, Address, Country, Phone - the first 4 lines.

Please, help me!!
