for problem 1
open this file
/includes/classes/db/mysql/query_factory.php put cursor right behind last ?> hit delete 10 times and save re-upload.

Problem2

when you installed the x-sell module you forgot to 1 add a file. or 2 the directions say to add a lines to includes/filenames.php

for 3.

you need to use the developers toolkit in the admin, this will show you the list of places to make this change
