Thanks to all! Once I figure out a way for this type of shipping to work, I will repost here in case anyone else has the same type of shipping they are setting up.

Basically one of the zones has to be zipcode based because of local delivery to a limited number of zipcodes in 2 different states (WA and OR) and the rest of the united states is being delivered from a different company.
That is the exact reason that I built this module. My customer has a shop in FL - she wanted to offer free shipping to those within 5 miles of the store if they spent over $50.00 and reduced for under $50.00 - Her regular shipping modules are installed for the rest of the world - So even if you live in FL but you don't live in one of the zipcodes in her list (I think she has about 7 or 8 - I would have to go look) you are automatically reverted to the standard UPS shipping rates, but if you do live in one of those areas - based on how much you spend you get the benefit of either free or reduced shipping.

So we have three options installed 1.- Store Pickup 2. UPS rates 3. Rate exceptions for those that want delivery but live close by.

For the rest of the world set up your shipping as you normally would. Just set this up for those exceptions as I have and it should work like a charm. Don't forget to modify your messages to reflect your particular situation in the appropriate language files. I've only provided the English file.
