Download a fax form in pdf?

How can I add a form i.e. Fax Order form in PDF to give clients the choice to order by fax?

Its NOT PDF but there is a fax form module in the downloads section

1 is for 1.2
the other is for 1.3

PO module, is it to the exclusion of other payment options (credit card, fax etc.)? Is it possible to specify wholesale level X to use PO , retail cc cards, wholesale level 1 fax ? I installed PO and now ceon cc option does not show on checkout.

forget the first part of my question, I got cc back.. but the second part - I still would like to know how to at least achieve limiting PO to a certain wholesale group..
