css and forms; extra atrrib. pictures

I cannot get any of the forms layout look right/good in purpleshades templates. (It works fine in other templates). I asked in the purpleshades, but no answer there. Where do I look to fix this? I also cannot get the extra images in attributes to show. (I get these 'console errors'):

Ok, to get the forms to match my apple zen template, add this to your css:
LABEL.inputLabel {
width:85%;
float:left;
}

LABEL.inputLabelPayment {
width:15em;
float:left;
}

To answer your ezpages question from your PM, ezpages aren't on on your site. You have the category tabs on though....

As to the attribute image, when I view source as you suggest, I don't see the HTML for the attribute image, it doesn't seem to be there.
