Problems with column grid layout contribution

Hello!

I have installed this mod on several different sites without a hitch, however I can't seem to get it to work on this site. I thought maybe it was because I set this database up with a table prefix, however I did some researching and if you use the SQL patch installer in the admin panel, then that shouldn't matter... however, I did try installing it in myphpadmin first and got errors, then tried to add the prefix, errors again. After that I loaded the SQL file in the tools > patch install and it said 2 things were loaded. I made sure to turn it on in the configuration > product listing... it's just not working. Did I screw up my SQL somehow?

Don't know what errors you are getting but the tables have to match.....so if the others have a prefix then the new one also has to have it

Here's what I get when I try loading it in again through the SQL patcher:
