    Why not login to your admin area, go to Configuration and click Minimum Values. Set all apart from First Name, Last Name, Email address and Password to 0 and then open the following file...
    includes\templates\template_default\templates\tpl_modules_create_account.php
    Remove the code for each of the boxes just leaving you with First Name, Last Name, Email Address and Password.
