Does anybody know where to download a Fax-Phone Credit Card module?

What do you mean by "fax-phone"?

Do you mean that, when a shopper checks out, he/she is offered a payment option called "FAX / PHONE your credit card details"?

If you want an option that says this, you can re-work one of the existing payment modules (eg: moneyorder).
