In the Image Handler Manager, are you putting a suffix in the field that says "Image suffix (optional)" ?
If so, don't. Just upload your image in the "default image file" field, making sure that the "bmz_cache" folder permissions are set to 777.

O.k. Kernel, don't get too excited, there......Go to Image Handlers' Admin section, and click on the "clear image cache", and then go back and try again.
What, may I ask, is the FULL name of the image that you're trying to upload?
