Add other purchase option at check out

Hi All,

How do I add another purchase option to the check out?

Giving the customer the choice of:
1) credit card
2) check
3) purchase order

I need to add a radio button for Purchase Order and blank box so the customer can enter the purchase order alphanumeric #.
