Make a simple form...

Hey there!,
This might be simple..but I have no idea!

I would just like to have a simple form in our contact us page.
For example a field that says perhaps:
Date costume required? (and a field for them to type in)

Is this easy?

The contact us info just goes into an e-mail as far as I know, so you could change the "Message:" text above the message area to say "Date Costume Required, Any Other Info:" or something.

To change the text, go to Tools > Developers Toolkit and paste Message: into the lower left box, select all files and search. Among the results you will find the file & line number where it is defined; open that file in a good text editor and edit.

While I'm at it, I'm trying to copy one of the standard fields to create another input field below say "email address" (in contact us)
I've got the box showing but not sure how to make the field function with correct name showing...
Ideas?
I'm playing around in "tpl_contact_us_default.php"
