If you visit our old contact us page at:

At the bottom of the page, the customer can request that we contact them for a number of different reasons. I need to possible put the same fields in either the contact us page, or the sign up page.

Please advise.

You can include these in your tpl_contact_us_default.php file

If you wrote the referenced page just plug in these into the tpl_contact_us_default.php file

Sorry... still not sure what you are talking about. I take it that I dont make these changes in the define pages editor. Do you have an example of what it is I need to do?

No example, but FTP the referenced file to your local and use a code editor or a plain text editor like "NotePad" (not a word processor), and add your code to this file where you want these selections.
