Create your form, ideally as a pdf or similar. Place the file in your site's download folder. The create a link to the file from your FAQ. How this link is created will depend upnj how your FAQ has been created. If it's using EZ-Pages and you're using one of the HTML editors then there will be a tool amongst the editors icons for doing so.
