I have installed and I'm using zencart for some days but I have a problem.

The site will sell different natural medicines (meds) so I need to have all the customers complete a medical test when they fill the signup form or afterwards they fill the sign-up form. This medical test is is fac a simple html page ( with some fields / radio buttons ).

How can I do this? Hope you can help me...
