attribute pull down with quanity

I have a client who wants something simila to the last entry on this website:

[pull down menu - $3.00] Quanity Box [ ]

[green]
[etc]

any ideas?

Have you actually tried adding to the cart from the example site? It doesn't work -

I does work if you change the boots to a color. If you leave it at the 'price $3.00 each) then it does not input a value.
