Anyways the COD problems in the wording.
According to Linda the Modules Order COD fee is different from the COD from payments. So
I set up my info as following:
Admin > Modules > Payments
I have Cash On Delivery Module Enable
Payment zone to none
Set order status to Update [4]
Sort order of dsiplay 0
then
Admin > Order Total Modules
I have COD Fee Enable
I set all the set to dollor amounts 0:00 which will give you no COD fee.
Note leave the wording ie
COD Fee for FLAT
AT:0.00,DE:0.00,00:0.00

COD Fee for Free Shipping by default
US:000

COD Fee for Free Shipping Module - (freeshipper)
CA:0.00,US:0.00,00:0.00

COD Fee for Free-Options Shipping Module - (freeoptions)
CA:.0.00,US:0.00,00:0.00

COD Fee for Per Weight Unit Shipping Module - (perweightunit)
CA:0.00,US:0.00,00:0.00

COD Fee for ITEM
AT:0.00,DE:0.00,00:0.00

COD Fee for TABLE
AT:0.00,DE:0.00,00:0.00

COD Fee for UPS
