Order Problem

I am having a problem with the orders on my store. When the customer checks out an e-mail is generated with the order number and price. There are a few problems.

1)I created the correct tax class, zone definitions, etc but the taxes are not being a applied.

2)I have a table rate setup for shipping. It shows up for the customer to select, but then it isn't applied to the total at the end. It doesn't show up in the order confirmation e-mail either.

3)The order is not appearing in admin. I can see that the customer has an order pending, but when I click on the pending link nothing is there.

I'm thinking this is a database issue, I am not sure what could be causing it though.
