Duplicate Gift Certificates being created

I have had two people purchase gift certificates today and it's creating two each time one is purchased.

Gives a different coupon/redemption code for each one.

This didn't used to happen, we recently upgraded the cart from 1.2.6 to 1.3.7.1 so I am guessing that is why, but have no clue as to the reason it's doing this.

Anyone know what I can do to check?

UPDATE:

I did notice that no model codes were entered for any of these gift certificates (I did not set these up). The instructions I have say it must start with GIFT (all caps). I did that, would that have caused this type of problem?

After your upgrade - did you uninstall and reinstall the Gift Certificates module?

I noticed a new column now, the Set Order Status (Set the status of orders made where GV covers full payment), I left that at default.
