client wants a feedback form

Hi there. Bascially my client wants a feedback form relating to to his services.
However he wants the feedback to be availiable to for others to view.

Im not quite sure what the best thing to do is.
The phpbb forum is too big and would more than likely discourage users from wrting reviews.

The reviews module which comes with zen cart only seems to be applicable to products, not services.

Can anyone sugguest a contribution which might do the job?
