These normally show by default, based on what you put in the Admin > Configuration > My Store > Store Address and Phone field.
Are you using a template that has removed them, or have you blanked them out in the Admin or when you installed originally?

I know how to display "My" information.
I need additional input fields for the "Customers" information.
Instead of just the Customer name with the comment box, I would like additional boxes for the customer to type in their own address, city, state, zip, fax information before they submit the contact form to me.
There is a MOD called "Add Customer Phone Number", I installed it and it added another box for the customet to enter their phone number. How about the address etc?
Assuming that you're talking about the "Add Phone Number to Contact Us!" mod, then all you need do is copy the changes that were made to integrate phone numbers and adapt them for the other fields. If you wanted the pulldown type interface for state, you could copy the code form the create account module built into Zen Cart. Other than that the only wrinkle is that for logged in customers your SQL statement would need to join the address book table to customers table to get the address information for the customers default address id.

Yes, I sort of thought that was the case. However, when I searched the various files to find the code to add to this mod I was confused with respect to the correct .php file I would grab the info from as it was not clear cut like the "PhoneNumber" field.
In addition, there were a few other pages that needed to be modified in the script as well. I will have to play around with it a bit.
