Ultimate SEO URL - category problem

Hi I installed seo url and now getting the following error
when I click on the category

select categories_name from categories_description where
categories_id=

Even the resulting path doesnt has the category name appended to the url
I edited the .htaccess to include the shop name i.e. "/"
Can someone help please

I have just observed that it works but when the cateogory cPath has a _ in it, it wont work
like
cPath=2_4

PS: I might have disabled the SEO url at the time when you click the url but this is for giving the idea
