I've been combing the forums looking for information on adding form fields. I've found a few results, but none that seem to match my needs. I want to add form fields in such a way that the customer is asked questions specific to the service they are looking at. Then, their answers should be emailed to me only IF they buy the service. Lastly, it would be nice to store the information they provided in their customer profile. Can this be done through attributes, or should it be added somewhere around the checkout area? Is this plausible, or am I undertaking a major overhaul?
